Resumo: Necrotizing periodontitis (NP), previously known as necrotizing ulcerative periodontitis, is characterized by extensive areas of tissue necrosis, gingival ulceration at interdental papillae, gingival bleeding, severe bone loss, halitosis and pain. The NP is considered the most dangerous category of periodontitis due to the fact that it develops and progresses quickly. Necrotizing periodontal diseases (necrotizing gingivitis and NP) are related to predisposing factors, including a compromised immune response of the host. The subject was a 21-year-old male patient that complained about "bad breath". During the clinical exam, periodontal pockets, gingival bleeding and gingival ulceration were observed. The patient was diagnosed in the early stage of NP. The non-surgical periodontal treatment of scaling and root plaining according to the protocol "one-stage full-mouth disinfection" plus antibiotics (7 days of 1500 mg amoxicillin and 750 mg metronidazole split in three daily doses) was performed. To this moment, the patient remains under supportive periodontal therapy.
