Resumo: Infantile malignant osteopetrosis is an uncommon life-threatening condition that requires prompt treatment to achieve favorable outcomes. Dysfunctional osteoclast formation and/or function results in excessive bone mass and density, leading to skeletal deformities and complications, such as loss of functional bone marrow, cytopenia, and infection. In 2016, a six-year-old male patient with this diagnosis and subsequent hydrocephalus and facial paralysis presented with suppurative lesions after mandibular tooth extraction. Hematological investigation revealed severe anemia. Due to the systemic condition, no surgical intervention was performed, and eventually the patient was referred to palliative care with antibiotics and blood transfusions every two to three weeks, which were interrupted in 2021 as the disease worsened. The severity of the infections resulted in spontaneous avulsion of the necrotic mandible through submandibular fistulas in 2023. Despite this dramatic event and subsequent maxillary infections, the patient did not pass away until 2024, following disease progression.
