Resumo: Mucous membrane pemphigoid is an immune-mediated disease that can present with oral lesions. A 47-year-old female patient reports cardiac arrhythmia and stress and was referred to our service, as post-covid she presented oral lesions, mainly on the gums, with painful symptoms. On intraoral clinical examination, diffuse erythema was observed in the gums with ulcerated lesions, secondary to vesicles. Positive Nikolsky test. An incisional biopsy was performed and the final diagnosis was mucous membrane pemphigoid. Systemic corticosteroids were prescribed and a G6PD enzyme test was requested, with results within normal limits. Therefore, in an attempt to reduce the corticosteroid dose, Dapsone was prescribed and sent to the periodontics clinic to control bacterial plaque. The patient has shown improvement in her clinical condition, especially after starting periodontal treatment. The importance of using Dapsone, when possible, and periodontal monitoring in severe oral manifestations of immune-mediated diseases is highlighted, especially in systemically compromised patients.
