Resumo: Hairy polyps are rare benign tumors in the oral cavity originating from ectodermal and mesodermal leaflets. It commonly appears in the nasopharynx and soft palate of children in the first years of life. We present a case of a 1-month-old newborn patient exhibiting a polypoid lesion in the anterior region of the hard palate with extra-oral projection. Following excisional biopsy, a fibrous consistency lesion measuring 3.5x1.4x0.6cm in its largest dimensions was observed. Microscopic examination revealed a polypoid fragment lined with stratified squamous epithelium, displaying either hyperplastic or atrophic characteristics, and variable keratinization. Additionally, differentiated skin appendages and lobules of mature adipocytes were observed, interspersed with fibrous septa, bundles of striated muscle, and blood vessels. Deeper sections revealed tubules with sebaceous characteristics, along with salivary gland acini adjacent to hairy structures. Remnants of bone were also evident. No recurrence of the lesion has been observed after surgical exeresis.
