Título: IMPACT OF "ACTIVE FOLLOW-UP" ON POTENTIALLY MALIGNANT ORAL DISORDERS AND ORAL SQUAMOUS CELL CARCINOMA IN TREATMENT

Resumo: OBJECTIVE: To analyze the results of an active follow-up in a subgroup of patients with OSCC-OPMD, characterizing clinicopathological variables and evaluating their impact on treatment.
METHODS: A retrospective study(1992-2023) was conducted on patients attending the Oral Medicine Service of a hospital in Argentina, with 5 years of follow-up for OPMD and OSCC. Demographic and clinicopathological variables and their impact on therapeutic regimens were recorded.
RESULTS: Nineteen patients were included, with an average age of 70 years (47-89), comprising 4 men (21%) and 15 women (79%). The initial diagnosis consisted of OPMD in 42% and OSCC in 58% of cases. Among precursor lesions, oral lichen planus was the most frequent (68.4%). The average follow-up duration was 11.6 years (5-32). A total of 144 biopsies were performed, averaging 7 biopsies per patient, with 21 histopathological reports of epithelial dysplasia and 87 malignant transformation events (carcinoma in situ 15.8%, OSCC 73.7%, and verrucous carcinoma 10.5%). Regarding treatment, of the 87 OSCC, 20% were treated under general anesthesia and 80% under local anesthesia. Additionally, 57.9% of patients received complementary oncological treatment.
CONCLUSION: Active monitoring facilitates early detection of OSCC, reducing mutilating surgeries, preserving organ function, improving patients quality of life, and reducing costs to the healthcare system.
