Resumo: A 43-year-old female presented with an asymptomatic lesion in the mental region detected on panoramic radiography. The patient had a history of 16 years post-orthognathic surgery involving Le Fort I osteotomy and genioplasty. The cystic lesion was surgically removed, and histopathological examination revealed a cavity lined with pseudostratified ciliated columnar epithelium and a cystic wall densely fibrous, devoid of inflammatory cells. Based on clinical, radiographic, and microscopic findings, the definitive diagnosis was mandibular surgical ciliated cyst. This rare entity underscores the potential implantation of respiratory epithelium in the mandible during orthognathic surgery. The identification of such cases is crucial for understanding the complexities of tissue reactions and potential complications associated with surgical interventions in the maxillofacial region.
