Título: PROGNOSTIC VALUE OF HISTOLOGICAL GRADING SYSTEMS FOR MULTISITE HUMAN EPITHELIAL DYSPLASIA: A META-ANALYSIS

Resumo: Objective: The current standard for assessing the malignant transformation risk of oral epithelial dysplasia through histopathological grading faces challenges of subjectivity and variability. This issue contrasts with the grading of dysplasia in other anatomical sites, where these difficulties are not commonly seen. This systematic review aims to evaluate various grading systems for epithelial dysplasia in oral, anal, penile, and cervical regions to ascertain their efficacy in predicting recurrence and malignant transformation. Methods: The review protocol was registered in PROSPERO (CRD42023403035) and followed the PRISMA checklist. A thorough search across major databases and gray literature sources was conducted. The Joanna Briggs Institute checklist assessed bias in individual studies. Results: Out of 46 eligible studies included, 45 underwent quantitative analysis. Meta-analysis indicated that binary systems exhibited superior predictive capacity for malignant transformation/recurrence in oral epithelial dysplasia compared to multilevel systems. Similarly, binary grading systems showed enhanced predictive accuracy in anal intraepithelial neoplasia. Conclusions: While no significant divergence emerged among grading systems for epithelial dysplasia across various anatomical sites, binary systems demonstrated superior clinical utility.
