Resumo: A one-month year-old girl was referred for consultation presenting a swelling in the maxilla. Her mother reported that the lesion was present since birth and was continuously growing, difficulting breast feeding. Clinical examination revealed a 2,0 cm, purplish, fibrous, pedunculated exophytic mass in the right upper anterior alveolar mucosa, covered by a smooth surface with an area of ulceration. Clinical diagnosis was congenital epulis of the newborn and the lesion was removed through a conservative approach. Gross analysis showed a blackened specimen. Histological analysis of the HE-stained slides revealed an atrophic stratified squamous epithelium and a proliferation of ill-defined large rounded to ovoid or poligonal cells with granular cytoplasm. Some cells were loosing their cytoplasm limits and vacuolization of the tissue was observed. Final diagnosis was infarcted congenital epulis of the newborn and the child is under clinical follow-up with no signs of recurrence for 4 months.
