Resumo: Objective: to compare the histological features, immuno-inflammatory infiltrate pattern, and expression of cytokines in minor salivary glands in Sjögren´s syndrome (SS) and nonspecific chronic sialadenitis (NCS). Materials and Methods: final sample was composed by 42 specimens from minor salivary gland biopsies obtained for SS diagnosis. Twenty specimens were diagnosed as SS and 22 specimens were diagnosed as NCS. Analysis included the pattern of the inflammatory infiltrate, acinar atrophy, ductal dilatation, fibrosis, and liposubstitution. Immunohistochemical reactions against IL-18, IFN-gamma, and TGF-beta 1 were performed. Results: the presence of lymphocytic foci was greater in SS (7,20) in comparison to NCS (0,59) (p<0,0001). There were no differences on the frequency of the histological and immunohistochemical parameters between the groups, except for the more frequent expression of IFN-gamma in cases without fibrosis. There was a tendency for a higher expression of IL-18 in SS, and expression of IFN-gamma in all cases and IL-18 expression in 22,7% cases of NCS. Conclusion: the pattern of infiltration of the immune-inflammatory cells was different in SS and NCS. In SS there was a higher expression of IL-18 and a lower expression of IFN-gamma, and IFN-gamma expression was higher in cases without fibrosis. Financial support: FAPERJ and CNPq, Brazil.
