Resumo: Odontogenic cysts and tumors are alterations associated with dental tissue development or the already constituted organ, composing an important in orofacial pathologies, considering their evolutions in the gnathic bones. A 26-year-old female patient, caucasian, attended the Oral and Maxillofacial Surgery service, reporting pain and pressure in theleft mandibular region for 15 days. Panoramic radiography detected an extensive expansile radiolucent lesion in the mandibular body extending to the condylar region, associated with a third molar displaced to the superoposterior portion. The patient had access to a previous examination 7 years earlier, during which a smaller radiolucent lesion was observed, restricting the mandibular angle region, associated with an impacted third molar. Due to the lesion extension, it was decided to perform an incisional biopsy and the marsupialization, and decompression of the lesion and extraction of tooth 37. The histopathological analysis of the obtained material confirmed a diagnosis of odontogenic keratocyst.
