Resumo: Fungi are opportunistic microorganisms frequently associated with various diseases affecting the oral and maxillofacial region. Their oral manifestations may result from either a primary or disseminated infection. A 64-year-old man was referred for evaluation of a lesion on the upper labial mucosa that had persisted for over a year. The patient experienced significant weight loss during this period. On extraoral examination, an edematous lesion was observed on the upper lip, and intraorally, multiple symptomatic superficial ulcers were identified in different anatomical sites. According to the observed characteristics, the clinical hypothesis of Histoplasmosis was primarily formulated. Two incisional biopsies were performed. Histopathological analysis led to the diagnosis of Paracoccidioidomycosis. Afterwards, the patient was referred for treatment at an infectious diseases specialized center. On his last follow-up, it was noted that the disease exhibited signs of resolution, and the patient continues in treatment and monitored regularly by the medical and dental team.
