Resumo: Madelungs disease (DM), also known as Multiple Symmetrical Lipomatosis, manifests with adipose tissue deposits, primarily in the head and neck region. A 42-year-old male presented with asymptomatic bilateral neck swellings evolving over two months, along with smaller nodules on the arm and chest. A cervical ultrasound was requested, and a fine needle aspiration biopsy was performed and sent for cytological analysis. Ultrasound examination showed bilateral subcutaneous deposits of unencapsulated fatty masses. Panoptic and Papanicolaou-stained cytological smears showed the presence of numerous adipocytes. Thus, the diagnosis of Madelungs disease was obtained through the association of clinical and cytological characteristics. It was decided to maintain only follow-up since the fat deposits are stable and do not cause aesthetic problems. The patient undergoes periodic clinical monitoring without the occurrence of new deposits or other changes related to DM.
