Resumo: The mandibular buccal bifurcation cyst (MBBC) is a rare inflammatory odontogenic cyst, primarily occurring on the buccal side of the first and second permanent molars in children. Its etiology and overall prevalence are poorly understood, though it is estimated to account for about 1% of all odontogenic cysts. This series reports seven cases of MBBC, describing their clinical, radiographic, and histopathological features that are crucial for diagnosis and therapeutic strategies. The cysts were more prevalent among female patients at a mean age of 8.15 ± 12.5 years (range: 6 to 11 years), who reported particularly buccal swelling near the lower molars and eventually pain. Radiographically presented as well-defined unilocular radiolucency with cortical bone expansion without perforation on cone beam computed tomography. Treatment typically involved surgical curettage with maintenance of the involved teeth, with no subsequent recurrences. Highlighting MBBCs unique characteristics underscores the importance of accurate diagnosis and effective treatment planning.
