Resumo: Objective: to analyze the architectural and cytological histological characteristics predictive of epithelial dysplasia in oral leukoplakias. Materials and Methods: all cases diagnosed as oral leukoplakias from 2005 to 2018, were selected. Clinical information was retrieved from the laboratory charts. Architectural and cytological histological parameters were obtained after histological review of all histological slides. Data was tabulated and submitted to univariate and multivariate statistical analysis considering the presence of epithelial dysplasia as the outcome. Results: the final sample was composed by 532 oral leukoplakias, with predilection to females (63,1%), adults (mean age of 59,9 years) and localized preferentially in the gingiva and alveolar mucosa (27,6%). Multivariate analysis showed that acantosis, irregular epithelial stratification, loss of polarization of the basal cells, "drop-shaped" epithelial projections, keratin pearls/dyskeratosis, superficial normal or abnormal mitotic figures, hyperkeratosis with epithelial atrophy, basal cells hyperplasia, variation in the nuclear and cell shape, increased nuclei/cytoplasm ratio, atypical mitotic figures, and hyperchromatism showed statistical significance. Conclusion: the architectural and cytological histological alterations observed in the multivariate analysis are predictive features of the presence of oral epithelial dysplasia.
