Título: OPPORTUNISTIC DIAGNOSIS OF POLYMORPHOUS ADENOCARCINOMA OF THE PALATE IN AN ELDERLY PATIENT: A CASE REPORT

Resumo: A 74-year-old female patient was referred for evaluation of an asymptomatic nodular lesion on the palate discovered during a routine dental examination. The intraoral examination revealed a 1cm telangiectatic nodule with fibroelastic consistency and normal colouration. The main clinical hypotheses included pleomorphic adenoma and mucoepidermoid carcinoma. A fine-needle aspiration biopsy was performed. Cell-block slides stained with hematoxylin and eosin, as well as exfoliative cytology stained with Papanicolaou and Panoptic, revealed basaloid epithelial cells arranged in a cribriform pattern. An incisional biopsy was performed, revealing fibrosis, scattered microcysts, and neoplastic cells with nuclear pleomorphism. Immunohistochemical tests showed: AE1/AE3+, CK7+, S100+, and SOX10+, and negativity for P63, Smooth Muscle Actin, and Vimentin. These findings favoured the diagnosis of polymorphous adenocarcinoma. Surgical resection with clear margins was conducted by a head and neck surgeon. Histopathological analysis confirmed the diagnosis. The patient has been under follow-up for 17 months, with no signs of recurrence.
