Resumo: Central giant cell lesion is a rare, non-neoplastic bone lesion. A 7-year-old male was referred to the Stomatology Department with a 1-year history of increased volume with facial asymmetry and decrease of the gingivolabial groove in the anterior mandible. Tomographic examination revealed an expansive osteolytic lesion with cortical rupture involving the lower anterior teeth, measuring 6 cm. He had a previous diagnosis compatible with a central giant cell lesion and non-surgical treatment with intralesional corticosteroid application, without response. Conservative surgery was performed, with the loss of lower incisors. The patient had a mandibular fracture and was followed until consolidation. Orthodontic treatment and rehabilitation with fixed prosthesis were performed and after 9 years, he underwent reconstruction surgery of the alveolar ridge with autogenous graft for placement of dental implants. The patient is under the 10 years of follow-up with no evidence of recurrence.
