Resumo: Benign cementoblastoma is a rare odontogenic tumor with a predilection for the posterior mandible. There is no gender predominance and most patients are in their second or third decades of life. Radiological image shows a radiopaque mass attached to the roots of the associated tooth. Treatment consists of complete surgical removal and prognosis is good. This report describes a 16-year-old male complaining of pain in the right posterior mandible lasting few months. Extraoral and intraoral examination showed no alterations. Conventional radiographs and computed tomography revealed a unilocular radiolucent image associated with the endodontically treated roots of the lower right first permanent molar. Clinical diagnosis was a periapical cyst. Conservative surgical removal of the lesion showed a solid mass and an osteotomy without removal of tooth 46 was performed. Final diagnosis was benign cementoblastoma and a 6-month follow-up showed no clinical alterations and evidences of bone formation in the area.
