Resumo: A 56-year-old woman referred to the Bucomaxilofacial service, with loss of implants in the region of element forty-four suggesting advanced periimplantitis, risk of pathological fracture in the anterior region of the jaw associated with the use of sodium alendronate demonstrating a diagnostic hypothesis of drug osteonecrosis evolving to osteomyelitis. The tomographic examination showed image suggestive of bone thickening, unconsolidated fracture without misalignment, dense foci and aspect of bone sequestration, increased density of spongy bone in the mentonian region and in the right hemibody of the mandible adjacent to the fracture, defining infiltrative nodular injury in the mandible. Subsequently performed sequestrectomy, referred bone sequestration to the anatomopathological analysis, with the result of metastatic carcinoma and suggestion of immunohistochemistry. Investigated primary tumor by oncology with immunohistochemical report of primary intraosseous carcinoma. Patient follows the treatment with five chemotherapy sessions and satisfactory result in remission and follow-up by oncology and Bucomaxilofacial.
