Resumo: A 56-year-old male patient, white, presented to the dentist complaining of discomfort in the upper left alveolar ridge. The dentist observed a reddish nodular lesion and recommended refraining from using the complete denture and scheduled a return for a biopsy. However, due to a significant increase in the lesion, he was referred for further evaluation. An erythematous, ulcerated pedunculated nodule measuring approximately 4.5 cm was observed. The CT scan revealed a hypodense area with irregular borders and dissolution of the vestibular cortical bone. An incisional biopsy was performed with a preliminary diagnosis of fibrosarcoma versus metastasis to be clarified. The histological and immunohistochemical diagnosis was compatible with undifferentiated pleomorphic sarcoma. The patient was referred to head and neck surgeon who performed the surgery. The diagnosis of high-grade undifferentiated pleomorphic sarcoma of the maxillary sinus was confirmed by the histopathological examination of the surgical specimen. The patient was referred for radiotherapy.
