Resumo: The diagnosis of histoplasmosis can be challenging, often mimicking other infectious or neoplastic diseases. Here, we present two cases of this uncommon fungal disease. The first case involves a 62-year-old white male patient with a history of prior diagnosis and treatment for paracoccidioidomycosis and histoplasmosis in larynx. He presented with a single asymptomatic 1.5cm ulcer in the palate persisting for 40 days. The second case involves a 71-year-old white man with a painful 10cm ulcer in the mandibular alveolar ridge persisting for one year. Considering the differential diagnoses of paracoccidioidomycosis, histoplasmosis, and squamous cell carcinoma, incisional biopsies were performed. Histological examination revealed granulomatous inflammation and the presence of small yeasts (PAS-positive). The final diagnosis was histoplasmosis, and both patients were referred to an infectious disease specialist for treatment and are currently under follow-up care. This case highlights the critical role of dentists in diagnosing oral lesions associated with histoplasmosis.
