Resumo: Developmental cysts include several entities that can occur in the oral and maxillofacial region. They vary according to their histopathogenesis, attached structures, clinical presentation, and can mimic other significant lesions. A 66-year-old leucoderm female was referred to the integrated ambulatory of stomatology, otorhinolaryngology and dermatology for the evaluation of oral alterations. Clinical examination revealed nodular lesions in the external mucosa of both lips, non-ulcerated, with fibrous consistency. The patient had arterial hypertension and was a smoker for almost sixty years. The clinical hypothesis included cutaneous horn, basal cell carcinoma or cyst. Excisional biopsies were performed. Histopathologic examination showed cystic lesions with orthokeratinized epithelium and fibrovascular capsule. The lumen contained keratin, tissue debris, cholesterol clefts and red blood cells. The superior lip lesion showed also hair shafts. The diagnosis was epidermoid cyst (inferior lesion) and dermoid cyst (superior lesion). After the surgical treatment, the patient remains under clinical follow-up.
