Resumo: A 32-year-old man presented a nodular and warty lesion on the posterior lateral border of the tongue accompanied by dysphagia and significant weight loss for over three months. Incisional biopsies were taken and microscopic examination revealed intense proliferation of spindle cells with hypercellularized and hypocellularized areas, sometimes hyperchromatic and pleomorphic. Findings of secondary infection for bacterial colonies and fungus hifles were also observed. The preliminary diagnosis was of nonspecific neoplasm of spindle cells suggesting a malignant pattern. After complementary immunohistochemical analysis, it was observed positivity for SOX10 and S100, and negativity for Desmin, SMA, and CD34 with a preserved expression of Hist. H3.3K27me. Based on these combined findings, the diagnosis of Schwannoma was established. For the treatment, the tumor was completely excised and the microscopic examination of the surgical specimen was highly suggestive of the Plexiform Schwannoma subtype. The patient is being followed up clinically, with no signs of recurrence.
