Resumo: A 5-year-old boy presented at the clinic with a toothache. Clinical examination revealed oral mucosa lesions, including white, homogeneous, non-scraping plaques on the tongue, tissue volume increases resembling collars, and hemorrhagic spots on the gingiva. Numerous painless ulcerated and erythematous lesions were observed on the hard palate. Extra-oral examination showed anonychia in multiple fingers and toes, nail morphological changes, and hyperkeratotic, scaly, and dry skin in areas prone to trauma. Family history indicated similar nail and skin lesions among relatives. Microscopic analysis of the tongue lesion revealed hyperkeratosis and acanthosis, while lesions on the palate showed subepithelial clefts filled with fibrinous material and phagocytic cells, dyskeratotic areas, exocytosis, and a subepithelial chronic inflammatory infiltrate. The combined clinical and microscopic findings led to the diagnosis of pachyonychia congenita. No treatment was prescribed for the oral lesions, and the patient was referred to a dermatologist. He continues to undergo regular clinical follow-up.
