Título: Recurrent extraosseous keratocyst in the right pterygomandibular region

Resumo: Odontogenic keratocyst is a cystic lesion that originates from remnants of the dental lamina. Although it is predominantly a central lesion of the mandible, it may very rarely occur in an extraosseous location. Only 12 published cases were found in the literature. In this clinical case we aim to report a patient with a keratocyst in an extremely atypical site, the pterygomandibular region. This is a 75-year-old male, complaining of discomfort in the "cheek" on the right side. On examination, a nodule was visible near the angle of the mandible. Puncture and aspiration were performed with the presence of purulent content and a computed tomography scan was requested showing a hypodense area whose biopsy led to the diagnosis of extraosseous keratocyst. Due to difficult access for ambulatorial management, the patient was referred to hospital for excision.
