Resumo: The buccal bifurcation cyst (BBC) is an inflammatory odontogenic cyst, most commonly found in the 1st and 2nd decades of life. It is generally asymptomatic, causing expansion of the buccal cortex and displacement of roots lingually. The BBC is clinically similar to a radicular cyst, except that the associated tooth remains vital. This study aims to report the case of a 13-year-old black male referred to the oral medicine/oral surgery clinic following radiographic examination for orthodontic planning. On intraoral examination, slight swelling was observed in the buccal aspect of the lower right molars. On tomographic examination, a bilateral unilocular hypodense lesion was observed in the furcation region of the teeth 36 and 46 with expansion of the buccal cortex and displacement of the roots lingually. Curettage of the lesions was performed, and the pathology report confirmed features compatible with BBC. The patient is under follow-up with no signs of recurrence.
