Resumo: AbstractOssifying fibroma is a rare neoplasm of odontogenic origin that may be associated with aneurysmal bone cyst. An 18-year-old black male patient, presented with an asymptomatic swelling on the left side of the mandible for four years. Radiographic examinations revealed a mixed lesion extending from the canine to the third molar with well-defined limits. With the hypothesis of central ossifying fibroma, a hemimandibulectomy was performed. The histopathological examination of the surgical specimen revealed a proliferation of spindle-shaped mesenchymal cells intermingled by irregular, immature bone trabeculae and globules of cementoid material. In other areas, extensive vascular spaces filled with blood intermingled with dense connective tissue and osteoclasts were observed. Given the characteristics observed, the diagnosis of central ossifying fibroma associated with an aneurysmal bone cyst was established. The patient was rehabilitated with autogenous bone graft and titanium plates and has been monitored for 14 months with no signs of recurrence.
