Resumo: Oncocytic sinonasal papilloma represents an infrequent benign epithelial neoplasm originating from the nasal sinus epithelium, with potential for malignant transformation. This investigation outlines the clinical presentation of a 44-year-old male patient presenting with nasal obstruction as the sole symptom. Magnetic resonance imaging revealed a hypodense, homogeneous mass obstructing the sphenoidal sinus, maxillary sinus, and nasal cavity, without evidence of osseous involvement. Histopathological examination disclosed respiratory epithelium supported by connective tissue, displaying multiple papillary projections. Chronic inflammatory infiltrate was evident within the connective tissue. Oncocytic characteristics were discernible in several regions of the epithelium, alongside mucous cells, microabscesses, and mucinous microcysts. The diagnosis was oncocytic sinonasal papilloma. The patient underwent endoscopic tumor resection, and given the absence of concurrent carcinoma, treatment was concluded. Subsequent follow-up at nine months post-treatment demonstrated a favorable recuperative trajectory with no evidence of lesion recurrence.
