Resumo: Neuralgia is characterized by pain propagating along nerve pathways, predominantly affecting the trigeminal nerve in the craniofacial region. Severe neuralgia presents with intense, episodic pain akin to electric shocks, often of uncertain origin. This study presents the case of an elderly male experiencing pain in the left mandibular branch, notably in the mental nerve. Upon diagnosis of neuralgia in 2016, treatment with low-power laser photobiomodulation was initiated. However, subsequent exacerbation of symptoms necessitated a biopsy, revealing traumatic neuroma yielding six months of pain relief. Pharmacological intervention with carbamazepine prescribed by the neurologist was met with intolerance, prompting discontinuation. The patient reported significant pain alleviation through biweekly photobiomodulation sessions, with symptom exacerbation upon cessation. Hence, continued adherence to this therapy is advised, given its efficacy in pain management. This case underscores the potential of photobiomodulation as a viable treatment modality for neuralgic conditions, particularly in cases resistant to conventional pharmacotherapy.
