Resumo: Surgical ciliated cyst is a rare complication following surgical procedures or trauma involving the jaws, primarily the maxilla. Over the past two decades, to the best of our knowledge, only 28 cases have been documented in the English literature. In this report, we present the case of a 52-year-old male who presented with a large unilocular lesion in the left partially edentulous maxilla. The patient disclosed a history of maxillary surgery several decades prior. Based on the clinical suspicion of a residual cyst, an excisional biopsy was performed. Histopathological examination revealed a cyst lined by pseudostratified ciliated columnar epithelium with areas of cuboidal and mucous cells. The definitive diagnosis was surgical ciliated cyst, and the patient is currently under observation. Although rare, it is important to consider surgical ciliated cyst as a potential diagnosis in cases of maxillary cysts, particularly in individuals with a history of facial surgeries.
