Resumo: An 11-year-old male presented with a tumoral lesion in the right maxilla, involving teeth 14,15 and 16 displaying a purplish color and lobulated surface. CT scan revealed a hypodense, unilocular image with broad expansion and thinning of the vestibular bone cortex. An incisional biopsy confirmed a diagnosis of a central giant cell granuloma. Conservative treatment involved weekly injections of 2.5 ml of Dexamethasone 4mg/ml (available at a public hospital) and topical intranasal Salmon Calcitonin twice daily, with a slow therapeutic response. Due to medication limitations during the COVID-19 pandemic, Triamcinolone injections replaced Dexamethasone, leading to clinical improvement after the first injection. Following 8 cycles of 6 fortnightly applications, a significant clinical response with reduced vestibular swelling was observed, reflecting a positive therapeutic outcome. After 4 years of treatment and monitoring, the latest CT scan showed a reduced lesion size, regular limits, and cortical periphery bone formation.
