Resumo: Odontoma is the most common benign odontogenic tumor, that can be divided into compound and complex, the latter composed of a disorganized conglomerate of dental tissues. It is usually asymptomatic, and the prevalence is higher in children. In this case report, a 17-year-old male patient presented an intra-oral volume increase causing tooth and inferior alveolar nerve displacement. A large hyperdense mass in the left posterior mandible, associated with an impacted first molar, was detected in a CT scan. The lesion caused expansion of the cortical bone and the patient reported mandibular pain. Initially, an incisional biopsy was performed and the microscopic examination showed the presence of dental tissue compatible with the diagnosis of odontoma. A complete surgical excision was performed. The histologic evaluation showed a disorganized dental tissue mass composed by dentin-like material, enamel matrix, cementum and dental pulp. The patient is under follow-up without signs of recurrence.
