Resumo: Exfoliative cheilitis (EC) is a rare inflammatory condition marked by excessive keratinization and periodic shedding of the lips. Research indicates nearly half of cases exhibit abnormal thyroid function. We present two instances of EC linked with thyroid disease. Female patients aged 20 to 54 presented with a two-year history of lip redness and burning sensation. Both had a medical history of hypothyroidism and unsuccessful prior treatments, including systemic and topical antimicrobial agents, corticosteroids, and cryotherapy ablation. Anamnesis, clinical findings and Histopathologic features suggested the diagnosis of EC. Treatment involved six months of methylene blue (0.01%) and red laser therapy (320 J/cm²), yielding significant improvement. However, complete resolution post-treatment was not achieved, necessitating ongoing observation. While uncommon, alternative therapies like cryotherapy and photodynamic therapy combined with PDT warrant consideration. These cases highlight the complex interplay between EC and thyroid disorders, urging exploration of diverse treatment modalities for comprehensive management.
