Resumo: A female patient, 75 years old, edentulous, sought dental care with severe pain in her right jaw for four days. During the anamnesis, the use of antiresorptive medications for the treatment of breast cancer was reported. In the extraoral and intraoral clinical examination, edema, redness, and fistula associated with the jaw and bone exposure were observed. The patient wore unadapted complete dentures. Cone Beam Computed Tomography was requested, which showed a pathological fracture. The final diagnosis was medication-related osteonecrosis of the jaw, with prosthetic trauma as the likely triggering factor. The proposed treatment was surgery to remove a necrotic bone segment and mandibular reconstruction. Laser photobiomodulation and ozone therapy sessions were carried out as a complementary treatment. After the surgical and complementary procedure, there was no pain or infection, and good intra and extraoral healing. After two months, the patient remains under clinical and radiographic follow-up without signs of recurrence.
