Resumo: Leiomyosarcoma is an extremely rare smooth muscle sarcoma in the oral cavity (1%) and is considerably aggressive. A 20-year-old male patient presented with mild facial asymmetry on the right side and an intraoral presence of a reddish, ulcerated tumor lesion on the mucosa of the left lower alveolar ridge. The lesion was sensitive, bleeding, fibrous in consistency and showed rapid growth in 10 days. Cone-beam tomographic examination only showed irregular resorption of the alveolar bone surface. An incisional biopsy was performed, revealing a proliferation of cells that were sometimes spindle-shaped and sometimes rounded, showing cellular atypia in a predominantly storiform and solid organizational pattern. An immunohistochemical study using AE1/AE3, S100, CD34, Desmin, αSMA and HHF35 markers was conducted, with only alpha-SMA and HHF35 being positive, indicating a diagnosis of Leiomyosarcoma. Tumor resection and chemotherapy were performed. Currently, he is under follow-up without evidence of recurrence or metastasis.
