Resumo: A case report detailing aflibercept-induced medication-related osteonecrosis of the jaw (MRONJ) is presented herein. A 64-year-old male patient with a history of intestinal cancer, liver and lung metastases, and treatment with chemotherapy and immunotherapy was referred to the School of Dentistry (August/2023) due to bone exposures. There was no history of antiresorptive use or other typical causes of osteonecrosis. Aflibercept, a monoclonal antibody with antiangiogenic properties, related to MRONJ (less than 10 cases reported so far), was among the medications administered. Clinical examination revealed two areas of bone exposure, ±2mm and ±1mm in size, on the lingual aspect of the lower alveolar ridge. Diagnosis confirmed stage 1 MRONJ attributed to aflibercept. Following initiation of the treatment protocol, partial regression of the condition was observed by the end of the follow-up period (September/2023). Given the increasing availability of biosimilar drugs, healthcare providers must maintain vigilance in managing such patients.
