Resumo: A 60-year-old woman complained of a black lesion on the palate that had been developing for two years. She has been monitored by dentists under a clinical diagnosis of physiological melanosis. Extra-oral examination was unremarkable. Intra-oral examination revealed asymptomatic black plaque, with irregular contours and surface, on the anterior region of the hard palate, extending to the alveolar ridge. Incisional biopsy was performed, and histological sections revealed a poorly differentiated malignant neoplasm composed by fusiform and epithelioid cells showing intense pleomorphism and atypical mitoses. Immunohistochemistry was positive for S-100 and MELAN-A, supporting the final diagnosis of melanoma. X-ray of the face and computed tomography of the skull revealed mucosal thickening in the maxillary, ethmoid and frontal sinuses, indicating tumor progression, and surgical treatment was ruled out. The patient underwent 35 sections of radiotherapy and was planned for chemotherapy. She died after 13 days of hospital admission, waiting for chemotherapy.
