Resumo: A 47-year-old white man presented a lesion on the edentulous alveolar ridge between teeth 43 and 47, associated with bone loss. A well-defined radiolucent, unilocular lesion was observed in the right mandibular body. After excision, histopathological analysis revealed epithelial proliferation in the form of cystic and solid follicles with extensive keratinization, similar with Odontogenic Keratocyst, and areas with ameloblastomatous epithelium. The diagnosis was Keratoameloblastoma (KA). Four years later, the patient returned with gingival growth in the same region. Following incisional biopsy, the diagnosis was Odontogenic Carcinoma (CO), and the patient was referred for treatment (surgery, radiotherapy, and cervical dissection). Given the diagnosis of KA and subsequent recurrence as CO, immunohistochemical characterization was performed. Tumor cells from KA and CO were positive for B-Raf. Ki67 positivity was detected in 30% of KA and 80% for CO. The patient has been followed up for 7 years and shows no signs of recurrence.
