Resumo: Warthin-like mucoepidermoid carcinoma (WLMEC) is a rare salivary gland tumor resembling Warthin's tumor. We analyzed 12 cases from Mexico (1), Guatemala (3), and Brazil (8). Their ages ranged from to 38-74 years (mean 57.5). Histologically, tumors display an oncocytic epithelial component intermixed with lymphoid stroma containing germinal centers. The cystic spaces contained mucinous secretions with mucous cells that were positive for PAS and mucicarmine stains. All cases were low-grade, except for one intermediate-grade tumor. Immunohistochemically, all cases were positive for CK5/6, p63, p40, EMA, CK7, and CK14, whereas only two cases showed S100 positivity. All cases were negative for GFAP and calponin. Molecular analysis revealed t(11;19)(q14-21;p12-13) with CRTC1(MECT1)-MAML2 fusion in all cases. Despite their benign appearance, WLMECs are low-grade carcinomas requiring complete excision. This multicenter study better characterized the clinicopathological spectrum of this uncommon malignancy. Increased awareness of WLMEC among pathologists and clinicians is crucial for accurate diagnosis and management.
