Resumo: Here, we report three cases of synchronous human papillomavirus (HPV)-related cervical and oropharyngeal non-keratinizing squamous cell carcinoma (SCCs) in women aged 34, 29, and 40 years. The oropharyngeal SCCs displayed a non-keratinizing morphology with irregular nests/cords of atypical squamous cells exhibiting high nuclear/cytoplasmic ratios, hyperchromatic nuclei, abundant mitotic figures, and individual cell keratinization. The cervical components showed analogous morphological features. Immunohistochemically, both tumor components were diffusely positive for CK5/6, p63, p40, and p16. HPV genotyping identified type 16 in two cases and type 18 in one case. The patients were treated with surgery and chemotherapy, as required. One patient died of the disease at 7 months; two patients remained alive at 14 and 11 months of follow-up. These cases highlight synchronous HPV-driven neoplasms occurring at distinct anatomical sites harboring identical viral types. Detailed clinicopathological characterization is essential to optimize multidisciplinary management.
