Resumo: This report aims to present the diagnosis and treatment of oral lesion associated with palmoplantar keratoderma. A 47-year-old female patient reported rare hereditary condition. Clinical exam revealed a whitish asymptomatic plaque on the dorsum of the tongue, 2.5cm in size, rough, asymptomatic, the past 10 months. Incisional biopsy was performed and histopathological examination indicated oral mucosa covered by parakeratinized stratified squamous epithelium and lamina propria with severe collagen deposition, compatible with leukokeratosis associated with the hereditary condition. The therapy adopted associated the daily practice of cleaning the lesion with coconut oil and a tongue scraper to reduce inflammation. Regression of the lesion was observed after 3 months of follow-up. Given the genetic syndromes associated with keratoderma, an accurate diagnosis is essential when choosing therapy. Use of coconut oil may be a promising therapeutic option due to its cost-benefit and anti-inflammatory potential, suggesting longitudinal studies to investigate biological plausibility.
