Resumo: A 62-year-old woman with hypertension and uncontrolled diabetes exhibited persistent mandibular pain for a month. Clinical examination showed swelling in the anterior lower alveolar mucosa. Cone beam computed tomography revealed denticles in the left parasymphysis and ectopic displacement of tooth 33 to the mental region, accompanied by a unilocular hypodense lesion attached to its crown. Following an excisional biopsy, microscopic analysis disclosed a cystic cavity lined by stratified squamous epithelium of variable thickness, containing PAS-positive mucous cells and microcysts, without inflammation in the fibrous capsule. The diagnosis was a dentigerous cyst and compound odontoma associated with an impacted mandibular canine. No complications or recurrence were observed during a six-month follow-up. The patient remains stable and under observation. The simultaneous presence of an odontogenic cyst and tumor in this case, unusual with few reports in literature, prompted a comparative study with other hybrid lesions, underscoring their rarity and complexity.
