Resumo: Objective: This study aims to evaluate the prognostic significance of NOTCH1 in patients diagnosed with adenoid cystic carcinoma (ACC) through a systematic review of existing literature and meta-analysis.Study design: Following the PRISMA 2020 guidelines, a thorough search of multiple databases was conducted in October 2023 without date restrictions. Inclusion criteria involved studies investigating NOTCH1-related factors at ACC diagnosis and their correlation with crucial outcomes such as recurrence-free or disease-specific survival. Meta-analysis was conducted on studies reporting hazard ratios (HRs) and sharing similar inclusion criteria and outcome measures.Results: Initial exploration identified 532 articles, with 9 studies meeting eligibility after rigorous screening. These studies, conducted between 2017 and 2022, were predominantly from the USA and China. NOTCH1 mutational status, Notch1 immunoexpression, and NICD1 immunoexpression showed significant associations with survival outcomes across all included studies. Methodological differences posed challenges in result integration. Meta-analyses of HRs were conducted with 2 studies for each index factor, revealing notable risk increases.Conclusion: This study underscores the prognostic relevance of NOTCH1 in ACC, suggesting its potential as a valuable risk stratification tool. Further research is necessary to validate these findings and clarify the clinical implications of NOTCH1 expression in ACC management. This study was supported by FAPESP 2023/17711-9.
