Resumo: Aim: To evaluate the distribution and association of clinical, radiographic and sociodemographic characteristics of non-syndromic (OKC) and syndromic (OKCs) odontogenic keratocyst cases and their recurrences. Study design: Cross-sectional and retrospective multicenter study of 1,103 OKC and 66 OKCs cases in which sex, age and presence of multiple lesions were analyzed in individuals seen at Brazilian oral and maxillofacial pathology centers. Of these, 1,341 lesions were analyzed regarding clinical diagnosis, size, site, imaging appearance, signs and symptoms, biopsy type, treatment and recurrence. Descriptive analysis was used for sociodemographic and clinical data. Chi-square, Fishers exact, linear association and Mann-Whitney tests were used for comparisons and correlations. Results: There was no predilection by sex. The median age of OKC and OKCs patients was 32 and 17.5 years, respectively. Unilocular lesions were more frequent. The posterior mandible was the most common site. Signs and symptoms were absent in most cases. Conservative treatment was the most common modality. Recurrences were rare. Conclusions: This study showed a higher frequency of OKC. Clinicopathological features may differ between OKCs and OKC cases. In addition, an association was found between the size of the lesion and some clinical characteristics, and between the time interval until recurrence and OKCs.
