Título: CONSPIRACY OF SILENCE IN HEAD AND NECK CANCER DIAGNOSIS: A SCOPING REVIEW

Resumo: Objective: To map and synthesize available evidence concerning diagnosis non-disclosure on head and neck cancer (HNC) patients.
Methods: A scoping review following JBI guidelines was conducted across major databases without period restriction, yielding 9,238 publications in English, Spanish and Portuguese. After screening and selection, a descriptive synthesis was conducted.
Results: Sixteen studies were included, primarily from Europe and Asia. Most studies (75%) were conducted in academic settings, involving a total of 662 patients, predominantly diagnosed with brain, oral, pharyngeal, and laryngeal tumors. Remarkably, 22.51% of patients were unaware of their diagnosis. Although physicians were the main source of diagnosis information (35%), they reported to often use vague terms to convey malignancy. Also, 13.29% of patients were aware of their diagnosis from sources other than doctors or caregivers. Caregivers (55%) supported diagnosis concealment, influencing physicians' actions. High diagnosis-to-death interval, education, age, and physician-specific factors significantly influenced diagnosis disclosure. Documented consequences involved psychological and psychiatric effects, including depression and anxiety.
Conclusion: Overall, HNC patients desire personalized open communication from physicians, but disclosure decisions are influenced by various factors. Research on the consequences of non-disclosure is limited, highlighting the need for further investigation in this area.
