Resumo: Toxic Epidermal Necrolysis (TEN) is a rare, potentially life-threatening mucocutaneous reaction often triggered by various medications, including antibiotics. We report the case of a 10-year-old male admitted to the pediatric ICU with TEN, manifesting with 80% skin involvement, and affecting ocular, oral, and genital mucosa, following exposure to benzathine penicillin three days prior. Oral examination revealed extensive, confluent, and hemorrhagic lesions covering 90% of the oral cavity. The patient required sedation, mechanical ventilation, orotracheal intubation, nasoenteral tube feeding, and central venous access. Antimicrobial photodynamic therapy (aPDT) and photobiomodulation combined with oral hygiene measures, were employed as treatment modalities. The patient showed complete recovery of oral mucosa and was discharged after 29 days. This case highlights the crucial role of laser therapy and oral medicine specialists in diagnosing and managing oral manifestations of TEN in pediatric patients.
