Título: An exacerbated case of alveolar osteitis

Resumo: Alveolar osteitis is a exodontia complication, occurring from 0.3% to 26%. A 46-year-old female smoker presented at an urgency attendance 3 days after 46 tooth extraction with intense pain. Intraoral socket presented without a blood clot and exposure of the lingual alveolar bone, pus secretion, and erythema of the surrounding mucosa. The patient underwent a management protocol with socket irrigation using saline solution, debridement of devitalized tissue, and application of an obtundent with zinc oxide guaiacol, followed by antimicrobial photodynamic therapy using 0.01% methylene blue and red laser 660nm with 100mW, totaling 9J of energy in the socket, for 4 sessions. Clindamycin 300mg and analgesics were prescribed. Total clinical regression and pain took 4 weeks. Early recognition and management of alveolar osteitis is important to prevent complications and promote patient recovery. Antimicrobial photodynamic therapy is an approach in the alveolar osteitis management, being effective and safe for infection control.
