Resumo: Extrapulmonary cutaneous tuberculosis, a condition rarely encountered, typically arises from direct inoculation or via hematogenous and lymphatic dissemination of the bacillus. This report delineates the diagnostic process for a 79-year-old male patient who exhibited a keratotic nodular lesion on the upper lip. An incisional biopsy was undertaken, with squamous cell carcinoma, fungal and mycobacterial infections, or leishmaniasis considered as differential diagnoses. Histopathological evaluation disclosed chronic granulomatous inflammation, although staining techniques for microorganisms-including Periodic Acid-Schiff, Grocott-Gomori, Ziehl-Neelsen, Fite-Faraco, and Giemsa-were negative. Serological tests for Paracoccidioidomycosis, Histoplasmosis, and Aspergillosis, in conjunction with culture and real-time PCR, did not detect Mycobacterium tuberculosis (MTB) in the sputum. The definitive diagnosis was confirmed through molecular analysis of the biopsy sample, revealing MTB presence.The commencement of antituberculin treatment resulted in the lesions complete remission, underscoring the pivotal role of direct oral lesion investigation in the diagnosis of significant infectious diseases.
