Resumo: The keratocyst is a distinct odontogenic cyst originating from the cellular remnants of the dental lamina. Depending on the treatment modality, it exhibits a potential growth mechanism and a high recurrence rate (2.2% to 95%). It shows a preference for males; 60% are diagnosed between the ages of 10 and 40, mainly located in the posterior body and ramus of the mandible (60% to 80%). A male patient, 23 years old and Caucasian, presented with inflamed gums in the left retromandibular region. Imaging examination revealed a multilocular radiolucent lesion extending from the branch to the body of the left mandible. An incisional biopsy was performed simultaneously with the extraction of the lower left third molar, showing nonspecific chronic inflammatory changes. Two years later, a new biopsy confirmed the diagnosis of a keratocyst. Hemimandibulectomy with a safety margin and a customized temporomandibular joint (TMJ) prosthesis were installed.
