Resumo: This study reports a case of a 58-year-old woman with chronic myeloid leukemia (CML), initially treated with Imatinib. The patient developed pruritic skin lesions on the upper limbs and painful ulcerated oral lesions, initially diagnosed as mucositis, unsuccessfully treated with photobiomodulation and photodynamic therapy. Due to a poor response to Imatinib, Nilotinib was initiated, which improved skin lesions but not oral lesions. At that time, treatment with dexamethasone was attempted, without improvement of the oral lesions. An incisional biopsy revealed oral mucosa partially covered by parakeratinized stratified squamous epithelium and lamina propria with chronic inflammatory infiltrate in the uppermost connective tissue, consistent with oral lichenoid lesion. Treatment with topical clobetasol propionate 0.05% improved lesions and pain, though some whitish plaques remained. Skin and oral lesions are common with Imatinib in CML, but those with Nilotinib are less documented. Early diagnosis is crucial for optimal therapy and managing the underlying disease.
