Resumo: Objective: Our aim was to analyze the histological subtypes of lip basal cell carcinoma (BCC) in order to better characterize its morphological features and potential implications for diagnosis and treatment. Study Design: Cases with histological diagnosis of BCC located in the lips between 1995 and 2023 were retrieved from three institutions. Morphological analysis followed the histopathological subtypes described by Niculet et al. (2022) and classified as high or low risk according to the WHO criteria. Depth of invasion (DOI) was calculated whenever feasible. Results: Seventy-four cases of BCC were examined, with the majority originating from the upper lip. The nodular subtype was the most prevalent, followed by micronodular, and infiltrative. Most cases were classified as low risk. Stromal inflammation was present in all cases and solar elastosis were present in 37 cases. DOI was analyzed in 37 cases. The potential overlap of BCC morphological features with other basaloid tumors of the lip and peculiar histological features can pose diagnostic challenges. Conclusion: Based on the results, it stands out that lip BCC is predominantly of the nodular type and unusual histological presentations were observed. The oral pathologist must be aware of the diverse morphological presentation of this entity to avoid misdiagnosis.
